Axolotl died unexpectantly

My Axalotyl Elvis died today, i have no idea why, he seemed alright to me about 2 hrs before i realised he was not alive, but in the passed he has been doing some really strange things like swimming around his tank with his head out of water and also would sometimes go a week without eating but would always come around and have a meal worm. I have had him for around 7 months now and this is the only things that were abit different, other then that he was a healthy looking axalotyl, no fungus or disease and the same in colour with big pink gills, i will be attaching a picture i took of him a little bit befor he died... please help me to work out why he died, i loved him very much and cried alot when i found him :cry:

What a shame, he looked lovely too.
Had you been keeping an eye on the water parameters, i.e checking for ammonia, nitrate, nitrites ? Swimming with his head out of the water usually indicates a problem with the water quality. How did you clean his tank ? With stones it can be a nightmare getting the gunk that gets trapped out. Do you know what the temperature was of the tank.Finally, where you feeding anything else other than meal worm? These are only really suitable as an occasional treat as their exoskeleton is hard to digest. Sorry for all the questions, but it may help with an answer.
 
I didnt know much about the water stuff, i liveon tank water that i pump up from the creek and the pet barn told me all i would need was a di-luder or something so thats what i have been using and have been since i got him, he only started swimming with his head out of the water for about 1.5 months or so, i would feed him meal worms and live fish i had tried a worm from the garden but he wouldnt have it, so i stuck to what he ate, sometimes he would eat like 6 fish in one night because i would just let them swim around his tank. I use to remove Elvis from the tank when i cleaned his, i would remove all rocks and ordimates and do a 30% change with a 100% change every now and then, i would re- clean everything in his tank and put him back and he would be fine. I cant believe i let my poor little Elvis down :(

    Would Chinese fire belly newts be more or less inclined towards an aquatic eft set up versus Japanese . I'm raising them and have abandoned the terrarium at about 5 months old and switched to the aquatic setups you describe. I'm wondering if I could do this as soon as they morph?
