Axolotl might be sick but I don't know why

I woke up to my axolotl Kirby having her gills white instead of pinkish. This really freaked me out so I moved her to a tub. She hasn't been eating well and floats to the top of the tank most times. She barely ever wants to eat the redworms I give her or the pellets. I think she may have also cut herself on the top of her gill because it is bleeding. I don't know what to do.
 
What are the water parameters? Is the water being dechlorinated? This looks like exposure to really bad water conditions.
 
This axolotl looks really poorly. Have you tried feeding earthworms? She needs to be kept in clean cool dechlorinated water. Change it daily and try to get her eating
